ABSTRACT

Metastasis is an infrequent cause of tumor involvement of the major duodenal papilla and of cholestasis. We report a case of obstructive jaundice due to a metastatic breast carcinoma to the major duodenal papilla, and assess curative and palliative treatment that interventional endoscopy can offer.


La metástasis es una causa infrecuente de compromiso de la papila mayor y colestasis. Reportamos una causa rara de ictericia causada por metástasis de cáncer de mama en la papilla mayor duodenal.

ABSTRACT

El carcinoma renal suele tener un comportamiento impredecible. Se caracteriza por la falta de signos clínicos precoces, por lo que en muchos pacientes se llega al diagnóstico en etapas muy avanzadas de la enfermedad, con diversas manifestaciones clínicas inespecíficas y presencia de metástasis a distancia. Es por esto que ha sido llamado tumor del internista. Incluso tras una nefrectomía, el tumor puede permanecer estable y metastizar años después. Cuando la enfermedad metastásica ocurre, suele ser múltiple. Presentamos el caso de un paciente previamente sano que debutó con hemorragia digestiva alta, en cuyo estudio se objetivó metástasis duodenal por adenocarcinoma de células renales avanzado, sin otras manifestaciones clínicas.


Renal cell carcinoma use to have an unpredictable behavior. It is characterized by the lack of precocious clinical signs reason; in many patients the diagnosis is made in stages very outposts of the disease, with diverse unspecific clinical manifestations and presence of metastases. That is why it has been called “the internist’s tumor”. Even after a nephrectomy, the neoplasm can remain stable and can metastasize many years later. When this happens, the metastases use to be multiple. We presented the case of a previously healthy patient who displayed upper gastrointestinal bleeding, in whose study we demonstrated duodenal metastasis by advanced renal cell carcinoma, without other clinical manifestations.

ABSTRACT

Small bowel neoplasms comprise 0.1% of all malignancies. Of these one-third are duodenal, mainly primary. Isolated duodenal secondaries arising on account of haematogenous spread is rare. Our case of duodenal obstruction due to an isolated duodenal metastasis as a result of haematogenous spread from breast carcinoma is unique in the literature.

ABSTRACT

Benefit of resection of metastatic lesions to the liver and lung from colonic cancer is well established. Resection of solitary metastasis or of locally recurrent malignancies in the periampullary region has now become the norm, as it increases survival. We present our experience with two patients with metastases in the periampullary region from previously treated colonic carcinoma who were treated with pancreaticoduodenectomy.

ABSTRACT

A case of Wilms' tumor with gastrointestinal tract metastasis of the duodenum was documented in a 22 month - old - child. She had been diagnosed with Wilms' tumor stage III, treated surgically and by radiation and chemotherapy about 1 year previously. While she was still on chemotherapy, she developed hematemesis and clinical signs of partial gut obstruction. Gastroscopy and upper gastrointestinal series showed an intraluminal vascular mass in the duodenal bulb and histologically proved to be Wilms' tumor. We believe this is the first report in the world of Wilms' tumor with gastrointestinal tract metastasis.

ABSTRACT

O diagnóstico de melanoma maligno metastático duodenal foi realizado em um homem de 35 anos com dor epigástrica. A biópsia confirmou o diagnóstico. Metástases no trato gastrintestinal podem aparecer muito depois do tratamento de tumor primário estra-abdominal e nao estao necessariamente associadas com metástases em outros locais. Em pacientes sintomáticos selecionados, o tratamento cirúrgico pode proporcionar prolongados períodos de paliaçao. O exame endoscópico está claramente indicado na avaliaçao de pacientes com melanoma maligno metastástico.

ABSTRACT

Se describe el caso de una paciente femenina de 68 años con tumor en cuadrante superior derecho del abdomen. El diagnóstico inicial fue de carcinoma del duodeno en base a los estudios de gabinete, incluidas las biopsias endoscópicas y trasoperatoria. La paciente falleció y en la autopsia se demostró que el tumor se originó en la vesícula biliar, lo que enseña la complejidad de su diagnóstico en los tumores avanzados .
